To provide the evidence base for improving the vitamin and mineral status of lactating women and their infants.
1b. Approach (from AD-416):
Measure vitamin and mineral status of lactating women and their infants, given a placebo vs. a micronutrient-rich, lipid-based supplement; existing samples will be used from 300 women-infant pairs in Malawi.
3. Progress Report:
This research contributes to Objective 2A of the in-house parent project. The Breastfeeding, Antiretroviral and Nutrition (BAN) study objective is the benefits of supplementing HIV-infected breastfeeding Malawian women with a peanut-based Lipid-based Nutrient Supplement (LNS). We have completed the analyses of matched maternal and infant plasma samples for vitamin B12, folate, ferritin, alpha-1 acid glycoprotein, C-reactive protein, transferrin receptors, and retinol binding protein (4300 samples). Tweleve thousand breast milk samples have been analyzed for vitamin A, thiamin, niacin, riboflavin and B6. We developed and validated a new method for measuring vitamin B12 in breast milk which is being used currently to analyze breast milk in this study and will result in two publications. Mineral analyses of breast milk will be completed in the next 6 months which will be the end of the sample analyses. Two manuscripts are in preparation on the effect of the antiretrovirals and micronutrient supplements on maternal and infant nutritional status. We intend to measure the mineral content of breast milk in the next 6 months which would be the end of sample analyses. Statistical analyses and manuscript preparation will continue to the summer 2013. Our development of new methods for breast milk content of micronutrients was a key factor in our obtaining the Bill and Melinda Gates Grand Challenges grant to evaluate breast milk composition in additional populations.
