Technical Abstract: Supplemental vitamin D is now widely recommended but little guidance is provided on how or when to take vitamin D supplements. The process of vitamin D replacement is complicated by the fact that the increment in 25-hydroxyvitamin D (25OHD) in response to a given dose varies greatly from person to person. This chapter will examine the factors associated with the 25OHD response to supplementation with vitamin D, including genetic determinants, the starting 25OHD level, body mass index, and gastric acid production. It will also consider the effects of a meal and of meal composition with respect to cholesterol and phytosterol intake and the amount and type of dietary fat on the 25OHD response to supplementation.
