Interpretive Summary: A single rectal ulcer is an uncommon finding in children with gastrointestinal disease. We described a patient with primary leukocyte adhesion deficiency (LAD) who presented with a fever, rash, and a rectal ulcer. Although inflammatory bowel disease (IBD) is foremost among the differential diagnoses, a primary immunological defect such as leukocyte adhesion deficiency should not be forgotten because of the different approach to intervention. As a result of our work, we are now aware that all ulcers should not be considered as correlated with inflammatory bowel disease.

Technical Abstract: A single rectal ulcer is an uncommon finding in children with gastrointestinal disease. Although inflammatory bowel disease (IBD) is foremost among the differential diagnoses, a primary immunological defect should not be forgotten. Because of the paucity of literature on the association of rectal ulcers and immunodeficiency disorders, we described a patient with primary leukocyte adhesion deficiency (LAD) who presented with a fever, rash, and a rectal ulcer.
