Technical Abstract: This questionnaire was used during quarterly, face-to-face interviews with the fifty-four Wisconsin dairy farmers who participated in the ‘On Farmers’ Ground’ nutrient management research project. It was designed to systematically and consistently compile information on herd size and composition, livestock facilities, land use, management practices, and motivations and goals related to feed, fertilizer and manure management. This questionnaire was designed for confinement dairy operations.
